Our office utilizes a system called CHADIS (Child Health & Development Interactive System) for some of the assessments and/or screenings performed at certain visits.


CHADIS is a secure, HIPAA compliant site that allows you to answer many of the developmental or behavioral questions that we ask during a well-child visit. Based on your child’s age or appointment type, CHADIS will send you a welcome email to create an account if there are any questionnaires that can be completed prior to your appointment. Click here to login.

What is CHADIS?

  • What is CHADIS?

    CHADIS is an online system that utilizes parent/guardian feedback to complete pre-visit developmental and/or behaaioral screenings. The CHADIS system sends the scored results of the completed screenings directly to your child’s medical provider to review and discuss with you during the well check visit.

  • Will I need to complete screening questionnaires before every appointment?

    Only the 9 month, 18 month, 24 month, & 30 month well visits currently require questionnaires to be completed. ADHD consults and followup appointments also require a questionnaire.  

  • Why is it important to complete the screening prior to the well check appointment?

    Reviewing the completed screening  or assessment results provides your child’s doctor with valuable information regarding their developmental progress or medication management that can be addressed during your checkup.


    Completing the surveys before your visit will avoid appointment delays and lengthy wait times.

  • Is the CHADIS Portal secure? Who can see my information and screening results?

    Yes! The CHADIS portal is secure and HIPAA compliant. Only your doctor and their authorized staff have access to your screening results and information.You will have a personal username and password to access the CHADIS questionnaires.

  • I have never used CHADIS before how do I log on?

    CHADIS sends out a registration email approximately 2 weeks prior to the patient’s appointment. Click on the link sent in the registration email to register and create an account within the CHADIS portal. Assigned questionnaires will be available in your account after registration completion.


    If you received the registration email and your child does not have a 9 month, 18 month, 24 month, & 30 month appointment coming up we encourage you to register for the program and write down your username and password for when questionnaires become available.

  • I have registered for CHADIS and now have questionnaires to complete?

    If you have already registered for CHADIS and receive an email saying you have questionnaires to complete, login with your created username and password to access your questionnaires. To login you can use the link sent within the email or go directly to the CHADIS website at www.chadis.com and login under the families and teens section.

  • If you do not have the initial registration email or you forgot your login information...
